Enoxaparin sodium is an anticoagulant medication commonly used to prevent and treat thromboembolic disorders, such as deep vein thrombosis (DVT) and pulmonary embolism (PE). It is a low molecular weight heparin (LMWH) that works by inhibiting factor Xa and, to a lesser extent, factor IIa (thrombin), thereby reducing the formation of blood clots.

Key Characteristics:
  • Administration: Enoxaparin is usually given via subcutaneous injection.
  • Dosage Forms: It is available in pre-filled syringes and vials.
  • Indications:
    • Prevention of DVT in patients undergoing surgery or those with restricted mobility.
    • Treatment of DVT and PE in conjunction with warfarin.
    • Prevention of thrombus formation in patients with unstable angina and non-ST elevation myocardial infarction (NSTEMI).
Mechanism of Action:

Enoxaparin enhances the activity of antithrombin III, leading to the inactivation of factor Xa and thrombin, which are crucial for the coagulation cascade.

Side Effects:

Common side effects may include:

  • Bleeding
  • Injection site reactions (pain, bruising)
  • Thrombocytopenia (low platelet count)
Monitoring:

Routine monitoring of coagulation parameters is generally not required, but renal function and platelet counts should be monitored in certain populations, particularly in those with renal impairment.

Contraindications:
  • Active major bleeding
  • History of heparin-induced thrombocytopenia
  • Severe renal impairment (with caution)
